POLICY PROCEDURE

1. INTRODUCTION

Bethel Home Health Services LLC will provide non-medical home services through home health aides, companions, sitters, and homemakers to clients in their places of residence. Qualifying services will be provided directly through agency employees.

Telephone answering service will be supplied twenty-four hours per day, seven days a week. On-call personnel will carry cell phones for contact in the event of an emergency.

The Agency Manager will coordinate services. This will include implementing, revising, and updating the Plan of Care/Service; scheduling of services, supervision of staff, and conferencing team members to plan and evaluate client care/service.

The qualifications and competence of the individuals providing service are appropriate to client needs, and the required services comply with applicable laws and regulations.

Bethel Home Health Services LLC will provide services to clients in the state of Ohio.

Agency maintains business hours from 9:00 am to 5:00 pm, Monday through Friday, except during holidays or as authorized by the Agency Manager.

Dissolution of Agency

In the event of dissolution of Bethel Home Health Services LLC, we will assure continuity of client care by adhering to client transfer or discharge notifications as follows:

  1. The client will be notified of the Agency’s dissolution.
  2. The notification will be documented.
  3. Agency closure will comply with State Agency Closure Procedures.

  • To develop a relationship with referral sources to effectively meet our clients’ home needs.
  • To function as a liaison between our clients and available community resources and assist our clients in coping with their needs and problems.

4. AGENCY OBJECTIVES

To provide:

  • A coordinated team approach to supportive care/services using home services workers and a system of referral, follow-up, and evaluation.
  • Home services and training, which allow clients and their caregivers to assume personal responsibility for clients’ personal needs.
  • Review vehicles for continuous examination of the care clients are receiving from the agency.
  • Educational opportunities within the home services system for employees through an ongoing in-service program.
  • Coordinated liaison with welfare agencies to meet the needs of the community through joint efforts, meetings, and community awareness programs.
  • A physical work environment conducive to maximum employee performance.
  • To recruit and retain highly qualified personnel.
